These moments can feel overwhelming, but remember: families ask because they care. Here are some tips for navigating these conversations:
✅ Stay calm – Even if you don’t have all the answers, your calm presence matters.
✅ Be honest – It’s okay to say, “I don’t know yet, but I’ll find out.”
✅ Offer reassurance – Let them know what steps you’re taking to help their loved one.
Your compassion and professionalism will make a big difference in the tone of the conversation and may help calm an upset family member.
REMEMBER: You won’t always have an answer they like and some people will stay mad. That’s just the reality unfortunately.

Here is a simple way to prevent med errors. It may be tempting to wait till your med pass is over to sign everything off.
But if you do that, you are seriously setting yourself up for a med error.
Also, the time stamp will be wrong which could also be a problem. Particularly when giving PRNs.

Top tips for administering medication’s through a peg or G – tube. This is the proper way to administer the medication‘s and the way you will need to do it when you are being audited by a state surveyor.
💊 Crush the medication is finally as you can, and dissolve with warm water.
💊 flush with about 30 cc of water in between each medication.
💊 certain medications cannot be crushed, make sure you are aware.
💊 look for opportunities to ask the doctor to change certain medications to a liquid form.
💊 always check for placement prior to administering meds also. Check with your facility for their protocol in doing this.
💊 if it’s clogged, don’t force push the liquid. Trust me…it will backfire- literally and you will get a bit of a shower 🤢 😂 been there done that.
What are your peg tube tips?
I just love this quote. Always look for the positive and focus on that. There is too much negative out there and focusing on it will bring you down. Even if you're having a terrible day at work, remember that it will end and will just become a memory.
When you write a progress note for a transfer out from a nursing home/rehab center, it’s important to have certain details.
What happened
What time they left
Where they went to (very important!)
Who was notified
Bedhold policy provided
Bedhold policy is a Medicare requirement to be provided to a patient when they go out to the hospital. If the patient is unconscious, just put it with the hospital paperwork so you can document that it was provided.
Then you can discuss with the family. Social services may help in this regard.

How to speed up your med pass. When you first start out it can be pretty hard if you don’t know the brand and generic names of meds. Work on memorizing those as well as which meds are kept in the fridge to minimize the time you spend looking for them.
Lots of times INR levels are messed up when a patient on warfarin starts an antibiotic.
They will usually need more frequent monitoring of their blood levels.
It’s very important to make sure the doctor is aware of this potential interaction.
Amiodarone is another medication that often has an interaction with antibiotics. Always make sure to check for allergies as well.
And don’t forget to write your progress note!

When your patient has low blood sugar it’s important to document each blood sugar that you take as their sugar is getting better. This way it’s easy to see how the interventions worked and how long it took for the sugar to become normal.

This is SUPER important. When a patient is refusing personal care, it can result in skin breakdown. There should be a record in the chart of the pattern of refusal a patient is having. The practitioner should also be aware as the reason behind the refusal needs to be investigated.
